During the initial charging of the CNG fuel tank, by lowering the temperature of the gas, cracks developed in the soluble alloy of the safety valve. Subsequent charging of the fuel tank in the field resulted in the cracks becoming penetrating cracks. If a vehicle is used under such conditions it is possible that gas can leak from the safety valve.

Some of the vehicles may have received a propeller shaft with insufficient high frequency heat treatment. Due to inadequate high frequency heat treatment on the base of the propeller shaft slide yoke spline, the propeller shaft may lack the designed strength and a crack may occur in this area.
