2014–2021 LEXUS Lexus GS-F (URL10R), RC-F (USC10R) GS-F (URL10R), RC-F (USC10R)

Due to a manufacturing defect, the low-pressure fuel pump may not operate as intended and could result in the check engine and master warning indicators to illuminate on the combination meter. If this occurs, it could impair the fuel supply to the engine leading to a loss of motive power whilst driving and inability to start the engine.

2025 LEXUS Lexus LX (FJA310R, VJA310R) LX500d F Sport, LX500d Luxury, LX500d Overtrail (4X4), LX500d Sports Luxury, LX600 F Sport, LX600 Luxury, LX600 Overtrail (4X4), LX600 Sports Luxury, LX600 Ultra Luxury

Due to a Transmission Control Module (TCM) programming issue, the transmission may rev faster than intended causing a loss of motive power and transmission fluid to leak. A transmission fluid leak in the presence of an ignition source could lead to a vehicle fire whilst driving.

2022–2024 LEXUS GX550 (VJA252R) & LX600 (VJA310R) GX550 (VJA252R), LX600 (VJA310R)

Due to a manufacturing defect, there is a possibility of engine machining debris contamination within the engine. This could lead to potential engine knocking, rough running and/or stalling during operation causing a loss of motive power.

2023 LEXUS RZ (XEBM15R)

Due to a software programming issue, the Heating Ventilation Air Conditioning (HVAC) electrical compressor may fail. If this occurs, the windscreen defrost function may not operate correctly and could lead to poor visibility for the driver.

2021–2023 LEXUS Lexus UX300e (KMA10R)

Due to a manufacturing defect, the electric heater element within the Heating Ventilation Air Conditioning (HVAC) system may deteriorate over time. If this occurs, the windscreen defrost function may not operate correctly and could lead to poor visibility for the driver.

2023–2025 LEXUS Various Models LBX (MAYH10R), LBX (MAYH15R), LM (TAWH15R), LM 350h (AAWH10R), LM 350h (AAWH15R), LS500 (VXFA50R), LS500h (GVF50R)

Due to a software programming issue, a memory device in the Combination Meter may deteriorate over time. If this occurs, the Combination Meter display may remain blank when the vehicle starts. This will cause a failure to illuminate the visual indicators, display of vehicle speed, engine status, and other critical safety information.

2024 LEXUS GX 550 (VJA252R)

Due to an error in the fitting instructions for the optional accessory Electric Brake Controller, the Lower Driver’s Airbag Assembly (removed during accessory fitment) may have been reinstalled using the incorrect torque. As a result, the lower driver’s airbag may not deploy as intended.

2023 LEXUS NX350 (TAZA25R), NX350h (AAZH20R, AAZH25R), NX450h+ (AAZH26R) NX350 F Sport, NX350h F Sport, NX350h Sports Luxury, NX450h+ F Sport

Due to a programming issue, the Adaptive High Beam System (AHS) LED units (high beam) and AHS ECUs (Electronic Control Unit) may incorrectly detect the signals for switching from low beam to high beam. If this occurs, a warning message will display and render the high beam inoperative.

Due to a manufacturing defect in the Adaptive High Beam System cracks may develop over time resulting in the LED reflector detaching from the reflector motor inside the headlamp assembly. If this occurs, the AHS function and automatic high beam may not operate as intended and a warning message will be displayed.

2021–2022 LEXUS LX600 (VJA310) LX600, LX600 F Sport, LX600 Sports Luxury, LX600 Ultra Luxury

Due to a design issue, the transmission control module may not operate as intended. If this occurs, the automatic transmission may continue to transfer engine power to the wheels when shifted to the neutral position. If the brake pedal or park brake are not applied it may result in unintended vehicle movement if shift lever is not moved to Park position.

2021–2022 LEXUS Lexus NX40h+ (AAZH26) NX450h+ F Sport 2.5L PHEV Auto CVT

Due to a manufacturing defect, the DC-DC converter that supports charging the 12V battery may short circuit and overheat resulting in a vehicle fire.

2012–2018 LEXUS Lexus IS, GS, GS F, RC, and RC F GS F (URL10R), GS200t (ARL10R), GS250 (GRL11R), GS300h (AWL10R), GS350 (GRL10R), GS350 (GRL12R), GS450h (GWL10R), IS200t (ASE30R), IS250 (GSE30R), IS300h (AVE30R), IS350 (GSE31R), RC F (USC10R), RC200t (ASC10R), RC350 (GSC10R)

Due to the design of the fuel tank vent tube assembly, there is a possibility it could develop a stress related crack. A stress related crack over time could expand to eventually allow fuel to leak from the crack. A fuel leak in the presence of an ignition source may result in a vehicle fire.

2021–2022 LEXUS Lexus NX250 (AAZA20R - 2WD), Lexus NX350 (TAZA25R - AWD)

Due to a software issue, the Electronic Parking Brake (EPB) may not operate as intended. This could result in the vehicle moving or rolling away if the gear selector is not in the “PARK” position.

2021–2022 LEXUS NX350h, NX250, NX350, NX350h & NX450h+ NX250 (Wagon), NX350 (F Sport Wagon), NX350h (Hybrid Sport Wagon, Hybrid F Sport, NX450h+ (Plug-In Hybrid Wagon), Wagon, Hybrid Luxury Wagon)

Due to a software issue in the Body Control Module (BCM), the Headlamp and the Daytime Running Lamps (DRL) may remain illuminated simultaneously for 60 seconds when using the Remote Connect Feature. The Remote Connect Feature is only available in the Lexus Connected Services smart phone application to help locate the vehicle by illuminating the headlamps. Note: This condition will revert to normal operation within 60 seconds.

2021–2022 LEXUS Lexus NX350h, Lexus NX250, Lexus NX350, Lexus NX350h NX250 (Wagon), NX350 (F Sport Wagon), NX350h (Hybrid Sport Wagon, Hybrid F Sport Wagon, Hybrid Luxury Wagon), NX450h+ (Plug-In Hybrid Wagon)

Due to a software issue, the Lane Tracing Assist (LTA) steering feature may not operate as intended. This may reduce the level of maintaining the vehicle’s position in the centre of a lane.

2021–2022 LEXUS Lexus LX & NX LX500d (Standard, F Sport, Sports Luxury),, LX600 (Standard, F Sport, Sports Luxury, Ultra Luxury), NX350 Hybrid (F Sport, Luxury, Sports Luxury),, NX450 Plug-In Hybrid (F Sport),

Due to a software issue, if the Vehicle Stability Control (VSC) system is turned off, it may not default to the ON setting the next time the vehicle is started. This may result in a loss of vehicle control. Warning lights will illuminate, indicating to the driver that the VSC is not activated.

2021–2022 LEXUS LX500d Diesel, LX600 Petrol, NX250, NX350, NX350h Hybrid, and NX450h Plug-In-Hybrid

Due to a software issue, the multi-media system may display video images while driving. This can result in driver distraction potentially drawing their attention away from the road.

2021–2022 LEXUS NX350h Hybrid (Luxury, F Sport and Sports Luxury) and NX450h Plug-In-Hybrid NX250, NX350, NX350 Hybrid, Luxury, F Sport and Sports Luxury, NX450 Hybrid Luxury, F Sport

Due to a manufacturing issue with the welding of the left and right front shock absorber mounting area body panels, the shock absorber may detach. This may result in a loss of vehicle control.

2015–2021 LEXUS RX & UX RX200t, RX300, RX350, RX450h, UX200

To meet the requirements of European towbar regulation ECE 55-01, it is necessary to apply an ECE label to the towbar when fitted to a vehicle. This label was not affixed to the towbar.

2020 LEXUS CT200h Luxury, Sports Luxury

Due to a manufacturing issue the resin weld between the fuel tank and fuel tank inlet pipe could peel off. This may lead to a fuel leak when the tank is full or being refuelled. A fuel leak in the presence of an ignition source may cause a vehicle fire. If this was to occur, vehicle occupants may identify a fuel odour when driving.

2013–2019 LEXUS Lexus ES300h, Lexus GS, IS, LX, LS, Lexus IS-F , Lexus LC, RX, NX and RC

The fuel pump on affected vehicles may stop operating. If the fuel pump stops working, warning lights and messages may display on the instrument panel, and the engine may not run smoothly. This can result in the vehicle stalling and the vehicle may not be able to be restarted.

2019 LEXUS Lexus CT200h MY2019

The gas struts that hold the back door open may be manufactured improperly, which may allow the gas charge to leak.

0 LEXUS IS 250, IS 350, ISF

Possibility that the replacement desiccated Takata airbag inflators fitted to the vehicles were manufactured improperly and may not fully inflate the front passenger airbag during deployment.

2019 LEXUS Lexus ES300h & UX250h

Some brake booster pumps may be manufactured improperly, and may cause the brake assist function to stop operating while driving.

0 LEXUS LC500h

The vehicles have improper programming in the ECU (controls the hybrid system and transmission), when the accelerator pedal is depressed quickly during normal driving conditions, the ECU commanding transmission may not shift gears within the required time period.

0 LEXUS GS-F, RC-F & LC500

The high pressure fuel pumps could be damaged under specific operating conditions. This damage can lead to abnormal noise and vibration and, over time, can cause the pumps to develop a fatigue crack.

0 LEXUS LX 470

Due to improper programming in the airbag control module initialisation sequence, there is a possibility that, under certain conditions shortly after startup, the Curtain Shield Airbags could activate when not necessary.

0 LEXUS 1S350

The bolts used to secure the housing and sprocket of the intake side Variable Valve Timing gear assembly could become loose due to abnormal impacts generated within the gear assembly immediately after start-up. The Variable Valve Timing gear housing and sprocket could separate and result in the engine stopping while the vehicle is being driven.

0 LEXUS RX400H

Due to variations in the circuit characteristics of the Intelligent Power Module, higher operating temperatures may result in internal solder degradation. Eventually this will cause heat damage to the Intelligent Power Module with illumination of various warning lights on the instrument panel. In most cases, the vehicle will enter a fail-safe mode, resulting in reduced driving power in which the vehicle can still be operated for short distances. In limited instances, the fuse of the power supply circuit could operate, causing the hybrid system to shut down and resulting in the vehicle stopping whilst being driven.

0 LEXUS RX400h

The inverter assembly is part of the hybrid system. The transistors on the control boards located inside the inverter assembly in some of the involved vehicles were inadequately soldered, and could be damaged by heat that occurs from large current flows that are typical under high-load driving conditions. If this occurs, various warning lamps will be illuminated on the instrument panel and in most instances the vehicle will enter a fail-safe driving mode, resulting in reduced power.

0 LEXUS IS250

Due to insufficient tightening of the fuel pressure sensor connected to the engine fuel delivery pipe with Nickel Phosphorus plating, there is a possibility that, over time, the pressure sensor could loosen.

0 LEXUS GRS300, GRS190

The fuel suction tube assembly is located in the fuel tank and incorporates the fuel pump and wiring power supply. The fuel pump wire harness insulation has insufficient flexibility and over time the internal connector may move resulting in wear and poor electrical contact.

0 LEXUS GS450h, LS460, GS460, LS600hL

The valve spring material may not have sufficient strength.

0 LEXUS LS460, LS600hL

On certain Lexus LS460 and LS600hL vehicles the Steering Control ECU programming for the Variable Gear Ratio Steering (VGRS) system can result in the steering wheel becoming off-centre by a maximum of up to 90 degrees under certain limited driving conditions such as when conducting a U-Turn or accelerating quickly following a sharp turn. If the driver has fully turned the steering wheel to full lock and then very quickly attempts to turn it back to the centre position, the steering wheel may temporarily become off-centred before automatically returning to the centre position within 1-5 seconds.

0 LEXUS IS250, GS300

During the manufacture of two fuel pipes located on the engine's fuel delivery manifold, high stress areas may have been created in the pipes. Combined with other factors such as continual high fuel pressure pulsations and the presence of natural corrosive agents in the fuel a fracture may form over time.

0 LEXUS IS250, GS300, 430

A possibility exists that if the front seat belts are extended quickly with strong force, the internal components of the Emergency Locking Retractor (ELR) may jam and the seat belt will not retract or extend any further.

0 LEXUS LS430

Due to improper moulding of the impeller of the electric fuel pump, alcohol contained in the fuel may cause impeller to distort and contact the fuel pump body resulting in seizure of the fuel pump.

0 LEXUS ES300

The lower portion of the Driver Side Airbag Cover (horn pad) may become detached from the airbag module during deployment. In the worst case, there is a possibility that the detached piece may strike the driver causing personal injury.
